A Word On Health Insurance...

Dear Prospective Patient,

Health insurance is a contract between you and your insurance company or between your employer and your insurance company. Your health care practitioner has nothing to do with the terms of this contract.

As a service to patients who choose to utilize health insurance, we provide benefit verification, bill submission, extensive documentation, and account management.

Accepting health insurance payment creates added expenses for our company. Excessive documentation and insurance verification requires extra time of our clinical and office staff. In order to receive payment for services rendered, an expensive contract must be maintained with a private billing company.

I frequently have patients request that I waive their co-pay or co-insurance payments. It is illegal to routinely waive co-pays, co-insurance, or deductibles. We do not waive co-pays, co-insurance, or deductibles unless proper documentation of financial hardship has been received, reviewed and accepted. (If you feel that you may meet the financial hardship guidelines, please call our office to request the appropriate forms, and we will gladly work with you.)

Remember, processing insurance claims is not easy. Many times payments are denied. Insurance verification, claims submission, and account management is a service provided to you by your health care practitioner, not a right. Ultimately, you are responsible for settling your account.

Due to the rising cost of co-insurance and co-pays, long waits for doctor visits, and high deductibles, many patients find our out-of-pocket payments hassle free and affordable.

We accept cash, check, Visa, MC, and health savings account payments.

You will receive a thorough initial evaluation, typically lasting one hour, for $90.00. All follow-up Physical Therapy appointments are priced at $45.00.

Fees are due at the time of service.
